Theoretically, Therapeutic Index (TI) is a quantitative measure of the relative safety of a drug, representing the relationship between the dose that produces toxicity and the dose that produces the desired therapeutic effect. It is founded on pharmacology and toxicology and provides an indication of the margin of safety associated with a medicine. Drugs with a high therapeutic index have a wide separation between effective and toxic doses, whereas drugs with a low therapeutic index require careful dosing and monitoring. In health economics, therapeutic index influences treatment selection, monitoring costs, adverse event management and cost-effectiveness analyses.
Mathematically, the therapeutic index is expressed as the ratio of a median toxic dose to a median effective dose or, in preclinical studies, the ratio of a median lethal dose to a median effective dose. The concept summarises the separation between efficacy and toxicity but does not fully characterise the shape or overlap of dose?response distributions. Consequently, therapeutic index is often supplemented by measures such as the certain safety factor or standard safety margin.
