Theoretically, Prevalence Difference (PD) is an epidemiological measure that quantifies the absolute difference in disease prevalence between two populations or exposure groups at a specific point or period in time. It represents the excess proportion of individuals with a condition associated with an exposure or characteristic and is founded on absolute comparison of prevalence measures. The concept exists to quantify the absolute burden of disease attributable to differences between populations.

Mathematically, the Prevalence Difference is represented as the arithmetic difference between two prevalence estimates. It is an absolute effect measure expressed in the same units as prevalence, typically as a proportion or percentage points. A positive value indicates higher prevalence in the exposed group, a negative value indicates lower prevalence, and a value of zero indicates no difference.

In practice, Prevalence Difference is estimated using cross-sectional studies, health surveys and surveillance systems. It is widely applied in epidemiology, public health and health economic evaluation to compare disease burden, assess health inequalities and quantify the absolute impact of exposures or interventions on prevalent disease.

A cross-sectional survey reports that the prevalence of chronic back pain is 18% among manual workers and 10% among office workers.

Prevalence Difference:

PD = 0.18 ? 0.10 = 0.08

PD = 8 percentage points

This indicates that chronic back pain is 8 percentage points more prevalent among manual workers than among office workers.
