Theoretically, Output Maximisation is an optimisation concept in production economics in which a decision-maker seeks to maximise the quantity of output that can be produced from a given set of available resources and technology. It represents the efficient transformation of inputs into outputs and is founded on production theory and constrained optimisation. In health economics, the concept underpins the efficient allocation of healthcare resources to maximise health service delivery or health outcomes within resource constraints.
Mathematically, output maximisation is formulated as a constrained optimisation problem in which an objective function representing output is maximised subject to production, budget or resource constraints. The mathematical framework determines the combination of inputs that yields the greatest attainable output while satisfying the specified constraints.
In practice, output maximisation is implemented using optimisation models, production functions and operational research techniques. It is applied in healthcare planning, hospital management and health system resource allocation to identify the most efficient use of limited staff, facilities, equipment and budgets.

Example

How does output maximisation relate to technical efficiency?
A unit maximising output from its inputs is technically efficient in the output-oriented sense, since no more could be produced from the same resources; any shortfall from the maximum is technical inefficiency. Output maximisation and input minimisation are two orientations of the same idea, that resources should not be wasted, viewed from opposite directions. Which orientation is used depends on whether inputs or outputs are taken as fixed in the situation being analysed.

When is output maximisation the relevant objective?
Output maximisation is the relevant objective where the inputs are fixed and the aim is to produce as much as possible from them, as when a provider's budget and staff are set and the question is how much care they can deliver. It suits settings in which resources cannot readily be varied in the period considered. Where instead the output required is fixed and the aim is to use the fewest resources, input minimisation is the appropriate framing.

What are the limitations of output maximisation as an objective?
Maximising output takes no account of whether the output produced is the most valued, so a unit can maximise output yet be allocatively inefficient by producing the wrong mix. It also says nothing about quality, so raising output can come at the cost of poorer care if quality is not held constant. As an objective it addresses technical efficiency alone, and pursuing it without regard to value or quality can mislead about overall performance.

How does output maximisation differ from cost minimisation?
Output maximisation seeks the most output from fixed inputs, a question of technical efficiency with resources given, whereas cost minimisation seeks the least-cost way of producing a given output, which brings in the prices of inputs and so concerns both technical and allocative efficiency in the use of inputs. Output maximisation holds inputs fixed and varies output; cost minimisation holds output fixed and varies the input mix to reduce cost. The two frame efficient production for different questions.
