Theoretically, Case Weight is a numerical value assigned to an individual patient episode that reflects the expected healthcare resources required to treat that case relative to a predefined reference case. It is a fundamental component of Diagnosis-Related Group (DRG) and case-mix funding systems, where each patient's diagnosis, procedures and clinical characteristics determine a relative weight representing anticipated treatment costs. The concept is grounded in health services research, hospital reimbursement and resource utilisation modelling.

Mathematically, a Case Weight is expressed as a relative coefficient, with the reference case assigned a value of 1.00. Cases expected to consume greater resources receive weights above 1.00, while less resource-intensive cases receive weights below 1.00. Case Weights are estimated using statistical analyses of historical hospital cost and utilisation data and form the basis for calculating reimbursement and measuring hospital case complexity.

In practice, Case Weights are used extensively within prospective payment systems to estimate expected costs, determine hospital reimbursement, calculate Case Mix Index values and compare healthcare providers after adjusting for patient complexity. Health economists also use Case Weights when modelling hospital expenditure, evaluating efficiency and estimating resource requirements.

Example

A hospital assigns a Case Weight of 1.75 to a patient's DRG. The standard reimbursement rate is �5,200.

Hospital Reimbursement = �5,200 ? 1.75

= �9,100

The patient's admission therefore receives a reimbursement of �9,100, reflecting its higher expected resource utilisation compared with the reference case.
