Concept Architecture
Concept
Theoretically, Average Length of Stay (ALOS) is a healthcare utilisation metric that quantifies the average duration of inpatient hospital admissions over a defined period. It represents an indicator of resource use, service efficiency and patient throughput, and is widely used in health services research, hospital management and health economic evaluation. The concept exists to summarise inpatient utilisation in a standardised manner, allowing comparisons across hospitals, specialties, interventions and healthcare systems.
Mathematically, Average Length of Stay is represented as the ratio of the total number of inpatient bed-days to the number of completed inpatient discharges during the same observation period. The measure provides an estimate of the expected duration of hospitalisation per admission and serves as an input into analyses of hospital costs, capacity utilisation, patient flow and economic modelling.
In practice, Average Length of Stay is calculated from routinely collected hospital administrative data, including admission and discharge dates or total occupied bed-days. It is commonly stratified by diagnosis, procedure, age group, provider or payer to evaluate efficiency, monitor service performance and estimate inpatient costs within health economic analyses.
Purpose
Used to measure average inpatient resource utilisation, evaluate hospital efficiency, estimate treatment costs, support capacity planning, benchmark healthcare providers and parameterise health economic and decision-analytic models.
Mathematical Formulae
Primary Formula
ALOS = Total inpatient bed-days Number of inpatient discharges
Supporting Formulae
Total inpatient bed-days = ? Length of Stay?
Length of Stay? = Discharge Date? ? Admission Date?

Example
A hospital records 480 completed inpatient discharges during one month with a total of 2,880 occupied bed-days.
ALOS = 2,880  480 = 6.0 days
The average inpatient admission therefore lasts six days. This value may subsequently be combined with average daily treatment costs to estimate hospital expenditure within an economic evaluation.

Frequently Asked Questions (6)
What is average length of stay?
A measure found by dividing total patient-days within a facility by discharges over the same period, an indicator of efficiency.

What average duration does average length of stay express?
Average length of stay expresses the mean duration of a hospital admission across a facility's patients, found by dividing total patient-days by the number of discharges over the same period. It boils many individual stays down to a single figure for how long, on average, patients remain. It serves as an indicator of efficiency, since shorter average stays, if outcomes hold, suggest care delivered without needless delay, while unusually long ones may signal problems. It aggregates the individual length of stay across many patients. The typical duration of admissions is what it expresses. The WHO's Everybody's Business sets out such measures.

How is average length of stay calculated?
Average length of stay is calculated by dividing the total patient-days within a facility by the number of discharges over the same period, so it gives the average stay per discharge. So average length of stay is calculated as patient-days over discharges, which is why it is an average, since it divides total days by discharges, and average length of stay is found by dividing total patient-days by discharges over the same period.

What does average length of stay indicate?
Average length of stay indicates efficiency, since a shorter average stay per discharge can suggest more efficient care, so the measure reflects how long patients typically stay relative to discharges. So average length of stay indicates efficiency, which is why it is used, since stay length relates to efficiency, and average length of stay indicates the facility's efficiency through the average stay per discharge over the period.

Why does average length of stay matter?
Average length of stay matters because it indicates efficiency: a longer average stay uses more resources per discharge, so the measure helps gauge how efficiently a facility uses its inpatient capacity. So average length of stay matters for efficiency, which is why it is tracked, since stay length affects resource use, and average length of stay matters because it indicates how efficiently a facility uses inpatient resources through the average stay per discharge.

How does average length of stay relate to length of stay?
Average length of stay relates to length of stay in that it averages it: length of stay is one patient's admission duration, and average length of stay is the total patient-days divided by discharges, averaging length of stay across patients. So average length of stay averages individual length of stay, which is why they are connected, since it aggregates them, and average length of stay is the facility-level average of the length of stay of individual patients over discharges.
