Theoretically, Relative Weight is a numerical coefficient that represents the expected resource requirements or costs of a healthcare service, patient case or Diagnosis-Related Group (DRG) relative to a predefined reference value. A relative weight of 1.00 represents the average reference case, while values above or below 1.00 indicate proportionally greater or lesser expected resource utilisation. The concept is grounded in health services research, prospective payment systems and resource-based costing.
Mathematically, a Relative Weight is calculated by comparing the expected cost or resource consumption of a healthcare service or patient group with that of a standard reference case. These coefficients are estimated using statistical analyses of historical utilisation and cost data and are used as multiplicative factors within reimbursement and resource allocation models.
In practice, Relative Weights are fundamental to DRG-based hospital payment systems, case-mix measurement and health economic modelling. They are used to calculate hospital reimbursement, estimate expected treatment costs, determine Case Mix Index values and compare healthcare providers after adjusting for differences in patient complexity and resource consumption.

Example
A DRG has an expected treatment cost of �9,600. The national reference case has an expected cost of �6,000.
Relative Weight = �9,600 � �6,000
= 1.60
If the hospital base payment rate is �5,500:
Hospital Payment = �5,500 ? 1.60
= �8,800
The patient episode therefore has a Relative Weight of 1.60, indicating expected resource requirements that are 60% greater than the average reference case.
