Theoretically, Real Option Value (ROV) is an economic concept that quantifies the value of preserving future opportunities under uncertainty. Adapted from financial option pricing theory, it recognises that decisions made today may create flexibility to exploit future information, technologies or interventions. In health economics, Real Option Value captures the additional value of extending life or delaying disease progression, thereby allowing patients to benefit from future medical innovations that are not currently available. The concept exists because conventional economic evaluations may undervalue interventions that preserve future treatment opportunities.
Mathematically, Real Option Value is represented using real options theory, in which healthcare decisions are treated analogously to financial options. Although several modelling approaches exist, including decision trees, dynamic programming and stochastic option pricing models, there is no single universally accepted canonical formula for estimating Real Option Value in health economics. Valuation depends on the probability of future innovations, the expected value of those innovations and the time over which future opportunities remain available.
In practice, Real Option Value is estimated using decision-analytic models that explicitly incorporate future treatment innovations and uncertainty. Analysts compare expected outcomes with and without access to future interventions, using scenario analysis, probabilistic sensitivity analysis or dynamic modelling to quantify the incremental value attributable to preserved future treatment opportunities. The concept has been applied particularly in oncology, rare diseases and rapidly evolving therapeutic areas.

Example
A new oncology treatment extends median survival by twelve months. During this additional survival period, a novel targeted therapy becomes available that provides an expected gain of 0.35 QALYs for eligible patients. Conventional cost-effectiveness analysis values only the current treatment benefit, whereas a Real Option Value analysis additionally incorporates the expected benefit arising from access to the future therapy, increasing the estimated overall value of the original intervention.
