Theoretically, Incidence Rate is the frequency at which new cases of a disease or health outcome occur within a population over a specified amount of person-time at risk. It is a fundamental epidemiological measure that accounts for both the number of incident cases and the duration of observation contributed by each individual. Incidence rate exists to quantify disease occurrence when follow-up times differ between individuals or when populations are dynamic.
Mathematically, incidence rate is represented as the ratio of new cases to the total person-time accumulated by individuals at risk. Unlike cumulative incidence, incidence rate is not a probability but a measure of disease occurrence per unit of person-time. It assumes that the hazard of disease remains approximately constant within the observation interval and is commonly expressed per 1,000 or 100,000 person-years.
In practice, incidence rates are estimated using cohort studies, disease registries, surveillance systems and clinical trials by recording both incident cases and follow-up time. Health economists use incidence rates to parameterise disease models, estimate transition probabilities, evaluate preventive interventions and project healthcare utilisation and disease burden.

Example
A cohort of 800 individuals is followed for varying lengths of time, contributing a total of 3,200 person-years of observation. During follow-up, 64 participants develop chronic kidney disease.
Incidence Rate = 64 � 3,200
Incidence Rate = 0.02 cases per person-year
Incidence Rate = 20 cases per 1,000 person-years
This indicates that 20 new cases occur for every 1,000 person-years of follow-up.

Why does an incidence rate use person-time in its denominator?
An incidence rate divides new cases by the total person-time at risk, the sum of the time each person is followed before developing the disease or leaving the study. Person-time is used because people are followed for different lengths, some joining late or dropping out early, and simply counting people would ignore how long each was actually at risk. Adding up their contributed time gives a fair denominator, so someone followed two years counts twice as much as someone followed one. It credits each person for their time at risk.
