Option-level NMB = (threshold × expected health outcome) − expected cost
INMB between two options = NMB of evaluated option − NMB of comparator
  • The same threshold and analytical framework must be applied to every alternative.
  • The option with the greatest expected NMB is preferred under the stated evidence and threshold.
  • INMB must name the two alternatives and the direction of subtraction.
  • The option with the greatest expected NMB may differ from the option with the highest probability of cost-effectiveness.
  • This educational tool uses expected values and does not model parameter uncertainty, budget impact or wider HTA considerations.
